We are always accepting new patients! In order to get started please fill out the medical release form and send to the office so we can request the necessary medical records from your Doctor's office.

Required Documents from Doctor's Office
1. Demographic Information
This includes the patients name, date of birth, street address, and phone number or best point of contact.
2. Insurance Information
Including the company name and member ID number.
3. Medical Information
The primary diagnosis and/or reason for referral, summary note of the most recent office visit, most recent labs to include A1c, glucose, eGFR, lipid panel (if available).
*For Children the most up to date growth charts are also required.

Occasionally Required Documents
Some insurance companies require a referral from the Doctor's office to be approved for nutrition counseling coverage. If this is the case we may request the referral from the Doctor's office using the document below.
